Steps To Home

Changing Lives…Improving Our Community…

STH accepts referrals for single adults and families from local shelters. Upon being accepted into the program, participants are placed in their own unit in the community. Intensive, concentrated case management is provided while they reside in FOCUS’s Program. FOCUS also works in conjunction with other agencies that support our participant’s needs.

Each person’s situation is assessed and an individual plan is designed. This plan takes the participant through three phases of development.

Steps to Home (STH) has 40 units of transitional housing. Housing units are located in the community. FOCUS pays the rent and utility subsidy while people pay a minimum of 30% on all housing related debt.

Phase One—Learning

This phase is up to six months. In this phase participants are involved in specific programming according to the participant’s needs, which are determined according to the participant and the case manager with direction from the supervisor.
Case managers provide intensive home visits two- three home visits per week. In this first phase Case managers are building relationships with the participant. This stage is full of learning not only for the participants but also the Case manager as well. The Case managers are linking participants to services in this phase. In phase one the emphasis is on:

Phase Two—Applying

This phase is up to 12 months. In this phase the participant is taking what they have learned and using the skills to make progress in the program. Participants in this phase are usually looking for employment and moving toward paying off housing related debts.

The case manager will continue to build on the programming and the things that the participant has learned in Phase One. In this phase case managers have intensive home visits 1-2 per week. The emphasis is on:

Ending Utility and Telephone assistance.

Continue payment on past due debts.

bullet

Advance Budgeting skills and techniques

bullet

Landlord and tenant mediation and education

bullet

Assistance with alcohol and drug abuse recovery

bullet

Referrals to other agencies

bullet

Referrals to SAFAH, Lucas County Metropolitan Housing Authority, Subsidized Housing and home ownership.

 

Phase Three- Self-Sufficiency

This phase is up to 6 months. In this phase the participants are moving toward self-sufficiency and independence from the FOCUS program. The participant is employed full or part time and the participant has completed all of their Individualized Service Plan Goals. The participant is moving toward stabilization. In this phase case managers have less intensive home visits 1-2 per month. The emphasis is on:

Referral to other housing options and programs.

Referral to FOCUS Aftercare Program.

Budgeting- allowing the participant to develop a budgeting system and plan for the future.

Completing payments on housing related debt.

Reduction of FOCUS program subsidy

Collaboration with other agencies
